Summary information for CPA reviews
Introduction
These notes list the information to be presented at the start of a CPA review, to summarise the current situation for a patient.
Mental health
Present very brief summary of psychiatric history to date highlighting any risk events.
Current mental state - level of symptoms and disability. Current medication - dose, side effects, compliance. Psychological treatment.
Physical health
Significant illnesses and extent to which they are problematic. Medication. Smoking status.
Social situation
Housing situation. Any contact with children and/or risk to children? Carer and/or caring responsibilities. Benefits including DLA, freedom pass. Debts.
Occupational situation
Work. Structured activities. Recreations. Social contacts.
Legal issues
Mention any court cases, immigration issues, etc.
